South End Community Health Center serves the greater Boston area and will mobilize a volunteer network of Latino Families to assist in reaching out to Latino and homeless community members. South End Community Health Center – awarded $304,385 CHOICE will also work to help families access related services across the system including social workers, school personnel and other support personal, such as those providing mental health services.
